Every visit is documented with measurable outcomes. Reassessments occur at regular intervals to adjust the plan and ensure the patient continues to improve.
When goals are met, the patient is discharged with a home exercise program and strategies to maintain their progress. If new needs arise, a new episode of care can begin at any time.
